Why this framework exists
Most dementia care is structured around diagnosis, risk and task completion. Care plans focus on what people can no longer do. Staff are trained to manage behaviour rather than understand it. The result is care that is safe but impersonal - technically compliant but disconnected from the individual.
Through years of specialist consultancy, education and direct practice across NHS Trusts, Local Authorities and care providers, Pat Hobson identified a consistent pattern: organisations wanted to deliver person-centred care but lacked a practical methodology to achieve it at scale. The Care Act 2014 requires local authorities to promote individual wellbeing. The Mental Capacity Act 2005 demands that individuals are supported to make their own decisions. The Human Rights Act 1998 protects dignity and autonomy. Yet without a structured framework, these statutory obligations remained aspirational rather than embedded in everyday practice.
The Unique Life, Unique World & Unique Environment™ framework was developed to close that gap - providing a structured, evidence-based approach that places the individual at the centre of every decision, from governance to the frontline.

The framework
Three interconnected dimensions that together create a complete understanding of the individual. Where the domains overlap, practice becomes genuinely integrated - the person's biography informs how their perception is understood, their perception shapes how the environment is experienced, and the environment either enables or restricts autonomy. Person-led outcomes emerge only when all three dimensions are held together.
Understanding the framework
Purpose
To understand the individual through their biography, identity, relationships, values and life story - preserving personhood as the foundation of every care decision.
Practical Meaning
Every person living with dementia has a lifetime of experiences, preferences, relationships and values that continue to define who they are. Unique Life ensures these are not lost to diagnosis but actively inform how care is planned, delivered and evaluated.
