Why PMH exists

Because Too Many Improvement Programmes Changed Nothing

PMH was founded because too many consultancy reports gathered dust and too many training days changed nothing. Pat and Mark Hobson saw organisations receiving advice that sounded impressive but could not survive contact with the reality of a ward, a care home, or a safeguarding investigation.

Rather than offering borrowed theory, they spent nearly three decades developing original frameworks, assessment tools and practical methodologies - directly from the challenges they encountered leading clinical teams, conducting investigations and supporting individuals with the most complex needs.

Today, those frameworks are used by NHS Trusts, Integrated Care Systems and care providers across the country. PMH's consultancy does not begin with a template. It begins with original thinking, developed through practice.

PMH's consultancy has been shaped by decades of frontline nursing, social work, clinical leadership and hands-on consultancy. That direct practice did not just inform our advice - it led to the development of original frameworks, assessment tools and practical methodologies that organisations continue to implement today.

Developed from direct dementia care practice

Born from Pat's frustration with behaviour-focused dementia care, this model reframes practice around the individual's biography, perception and environment. It has since been implemented across residential, nursing and acute care settings to achieve sustained reductions in distress and restrictive interventions.

Connects board-level safeguarding assurance directly to day-to-day frontline practice. Designed after Pat identified that governance failures rarely originated at the top or the front line alone - but in the gap between them. This framework creates auditable, sustainable improvement across the entire pathway.

Improvement and Compassion Fatigue

Developed from repeated transformation failures

A diagnostic model that identifies why previous improvement initiatives have stalled or collapsed. It helps organisations understand the organisational, cultural and emotional factors that prevent change from embedding - enabling them to build sustainable improvement without repeating past failures. This is based on a trauma-informed approach and practice and draws from the key principles of trauma-informed approaches.

Mark Hobson

28years of
experience

A Social Worker of 28 years and a Mental Health Act specialist, Mark works across mental health, forensic mental health, prison settings, physical disabilities, children and families, older people and dementia care. A former advanced practitioner, Approved Mental Health Professional and Form F Assessor, he advises organisations on the Mental Health Act, the Mental Capacity Act and Care Act safeguarding - from assessment and investigation through to Court of Protection proceedings and Section 21A challenges. He works alongside Pat's consultancy practice where cases intersect with distress, risk and contested decision making.
